Minority Community Health Partnership HIV/AIDS Demonstration Grants FY 2006 Grantees This program seeks to improve the health status relative to HIV/AIDS, of targeted minority populations through health promotion and education activities. This program is intended to test community-based interventions on reducing HIV/AIDS disparities among racial and ethnic minority populations, and demonstrate the effectiveness of community-based partnerships involving non-traditional partners at the local level. |

Youth Empowerment Program Grants FY 2006 Grantees This program is designed to address unhealthy behaviors in at-risk minority youth, and provide them opportunities to learn about more positive lifestyles and enhance their capacity to make informed and healthier life choices. It is intended to test community-based interventions on reducing risky behaviors among targeted minority youth. These demonstration grants require a multi-partner approach involving institutions of higher education, primary and secondary schools, community organizations and institutions, and the community at-large. |

Technical Assistance and Capacity Development Demonstration Grant Program for HIV/AIDS-Related Services in Minority Communities FY 2005-2007 Grantees Grant recipients will identify community-based minority-serving organizations that are well linked with minority populations affected by HIV/AIDS, and which have recognized needs and/or gaps in their capacity to provide HIV/AIDS-related prevention and care services. |

Community Programs to Improve Minority Health FY 2004-2006 Grantees The purpose of the Community Programs to Improve Minority Health Grant Program is to improve the health status of targeted minority populations through health promotion and disease risk reduction intervention programs. The intent is to demonstrate the effectiveness of coalitions in carrying out projects that coordinate integrated community-based educational screenings and outreach services, include linkages for access and treatment to minorities in high risk, low income communities, and address sociocultural and linguistic barriers to health care. |
