FY10 State Partnership Grant Program to Improve Minority Health
The purpose of this State Partnership project is to reduce health disparities in the minority populations in Wyoming by addressing access to health care, obesity and overweight. Many minority communities, particularly Native American and Latino communities, lack access to health care. Within these communities there is a lack of awareness and knowledge about the connections between diet, exercise and obesity. In order to address these issues, the project will update the Wyoming Heath Disparity State Plan and Wyoming-related information about health disparities, build a greater awareness about health disparities in Wyoming communities and among health care service providers. A toolkit will also be developed and disseminated to Wyoming service providers in order to improve culturally and linguistically appropriate service. After implementing these interventions, the project hopes to improve planning, coordination, collaboration and linkages among partners in the state, as well as increase state efforts to improve minority health. In addition, the project hopes to eliminate health disparities through the support of community programs that promote science-based health promotion and disease prevention research.
